    Minimally Invasive Surgery for Intraocular Lens Removal and Intrascleral Intraocular Lens Fixation with Trabeculectomy in a Patient with Dislocated Intraocular Lens and Elevated Intraocular Pressure

    A 88-year-old female who was being treated for end-stage pseudoexfoliation syndrome was referred to our hospital for treatment of dislocated intraocular lens (IOL) and the elevated intraocular pressure (IOP) and in the right eye (RE). At the first visit to our hospital, best-corrected visual acuity (BCVA) was 0.2 in the RE and 0.02 in the left eye (LE). IOP was 47 mm Hg in the RE and 21 mm Hg in the LE. Slit-lamp examination showed no abnormalities in anterior segments and dislocated IOL in the RE. Fundus photograph showed optic disc pallor in both eyes. We performed the combined therapy of flanged intrascleral IOL fixation with the double-needle technique and trabeculectomy. Throughout the follow-up period, BCVA slightly improved from 0.2 to 0.4 in the RE. The angle of tilt of the IOL was 6.6, 7.9, and 8.7° as measured by swept-source optical coherence tomography at 1, 4, and 6 months after the surgery, respectively. The IOP remained less than 10 mm Hg without having to administer any other glaucoma medications. Furthermore, any complications associated with the surgery were not confirmed

    Helical Micro-Hole Drilling Of Chemically Strengthened Glass Using Capsule-Shaped Electroplated Diamond Tool

    This study investigates the micro-hole drilling performance of chemically strengthened glass plate by using a capsule-shaped electroplated diamond tool and the helical drilling method. Three different helical pitch conditions were tested to drill holes with a diameter of 1 mm. The number of drilled holes, grinding force, and maximum crack size were measured along with the observation of the drilled holes to evaluate the performance of the micro-hole drilling. From the experimental results, it was found that as the size of helical pitch decreased, the number of drilled holes increases where the average grinding force generated becomes smaller. By using small helical pitch condition, 43 holes could be drilled but the maximum crack size generated at the outlet side of the drilled hole is not able to achieve the high-grade quality compared to the inlet side. The resultant grinding force generated when the tool tip nearing the outlet side of the glass plate has caused the large crack at a certain position on the outlet side

    Endoscopic resection of pleomorphic adenoma

    Background : An accessory parotid gland (APG) is a common anatomical structure that occurs in 10%–56% of individuals. Pleomorphic adenomas are the most common benign tumors of the APG, and their ideal treatment is surgical excision, although there is a risk for aesthetic disorders and facial nerve damage due to the site of origin. Moreover, despite being benign, these tumors are known to recur. Therefore, it is necessary to achieve both reliable excision and avoidance of facial nerve damage. Case presentation : We report a case of a 49-year-old Japanese man with a mass in his left cheek. The lesion was diagnosed as a benign salivary gland tumor derived from the APG by computed tomography imaging, magnetic resonance imaging and fine needle aspiration cytology. We resected the tumor using modified high submandibular incision under the endoscopic-assisted field of view. Discussion and Conclusions : The tumor was less invasive and reliably resected using an endoscope. In surgical treatment, the endoscopic-assisted technique is very useful to achieve complete tumor resection and prevent relapse while avoiding serious complications due to surgical procedures

    Optimization of Wet Grinding Conditions of Sheets Made of Stainless Steel

    This study addresses the wet grinding of large stainless steel sheets, because it is difficult to subject them to dry grinding. Because stainless steel has a low thermal conductivity and a high coefficient of thermal expansion, it easily causes grinding burn and thermal deformation while dry grinding on the wheel without applying a cooling effect. Therefore, wet grinding is a better alternative. In this study, we made several types of grinding wheels, performed the wet grinding of stainless steel sheets, and identified the wheels most suitable for the process. As such, this study developed a special accessory that could be attached to a wet grinding workpiece. The attachment can maintain constant pressure, rotational speed, and supply grinding fluid during work. A set of experiments was conducted to see how some grinding wheels subjected to some grinding conditions affected the surface roughness of a workpiece made of a stainless steel sheet (SUS 304, according to Japanese Industrial Standards: JIS). It was found that the roughness of the sheet could be minimized when a polyvinyl alcohol (PVA) grinding wheel was used as the grinding wheel and tap water was used as the grinding fluid at an attachment pressure of 0.2 MPa and a rotational speed of 150 rpm. It was shown that a surface roughness of up to 0.3 μm in terms of the arithmetic average height could be achieved if the above conditions were satisfied during wet grinding. The final surface roughness was 0.03 μm after finish polishing by buffing. Since the wet grinding of steel has yet to be studied in detail, this article will serve as a valuable reference

    Polishing Performance of a Recycled Grinding Wheel Using Grinding Wheel Scraps for the Wet Polishing of Stainless-Steel Sheets

    The surfaces of large austenitic stainless-steel sheets, which have side lengths of at least 1 m a sheet thickness of at least 6 mm, used for food tanks and sliding plates in seismic isolation devices, must be finished to a mirror surface. Polishing is performed to improve the surface quality of such sheets and dry machining is typically applied. The problems associated with dry machining are the exhaust heat of machining and treatment of chips. A transition to wet machining is required to solve these problems. In our laboratory, we have developed a wet polishing machine and researched the selection of grinding wheels to develop wet polishing technology for large stainless-steel sheets. In this study, to reduce tool cost and reuse resources, we attempted to manufacture a recycled grinding wheel using snippets of grinding wheel scraps. A polyvinyl alcohol (PVA) aqueous solution was used as the bonding agent for the recycled grinding wheel to reduce environmental load. To overcome the ease of dissolution of PVA in water, we attempted to improve the water resistance of the PVA aqueous solution by incorporating an organic titanium compound. This is one of our efforts to contribute to sustainable development goals. The results are summarized below. (1) A recycled grinding wheel was fabricated by kneading crushed pieces of grinding wheel scrap with a bonding agent. (2) The maintenance of the shape of the recycled grinding wheel was controlled by the concentration of the bonding agent. (3) The recycled grinding wheel with a PVA bonding agent was vulnerable to water. In contrast, the recycled grinding wheel to which the organic titanium compound was added exhibited improved water resistance. (4) The polishing of stainless-steel sheets using the plain PVA recycled wheel was relatively ineffective, but polishing using the recycled wheel with the titanium additive was comparable to polishing with a new grinding wheel

    Clinical Course of OCTA en face Imaging Findings in a Patient with HELLP Syndrome

    A 36-year-old primigravid woman with suspected gestational hypertension was referred to our hospital for further examination and treatment of bilateral serous retinal detachment (SRD). At the previous hospital immediately after the onset of visual symptoms (decreased visual acuity), the best-corrected visual acuity (BCVA) had been 0.1 in the right eye (RE) and 0.08 in the left eye (LE). Funduscopy revealed diffuse SRD and white discoloration at the posterior pole but no retinal tear in both eyes (BE). Optical coherence tomography (OCT) also revealed SRD. One day after the onset, we diagnosed bilateral SRD due to preeclampsia with HELLP syndrome because of elevated blood pressure, hemolysis, elevated liver enzymes, and low platelet and performed an emergency cesarean section. One week after the onset, BCVA improved to 0.3 in BE. Funduscopy showed resolution of SRD and the remaining white discoloration at the posterior pole. OCT showed complete resolution of SRD, blurred external limiting membrane, and an invisible ellipsoid zone. OCT angiography (OCTA) segmentation of the deep retinal layer (using en face imaging) revealed multiple white materials, mainly in the macula. Although funduscopy and OCT showed almost normal appearance by 2 months after the onset, BCVA was not completely improved. OCTA using en face imaging revealed shrinkage of multiple white materials but remnant material at the posterior pole. One year after the onset, BCVA was 0.9 in the RE and 0.8 in the LE.
